My PSA was 3.9 to 15 in 17 months, then increased to 18.3 in 7 weeks, what should I do?

Like all health professionals Me saying: "Do not worry, cancer of the prostate cancer is increasingly slow …", my PSA rose from 3.9 to 15 in 17 months and rose 7-week weeks after the PSA test result above. Online research, I discovered that I have an aggressive type of prostate cancer and should be treated as soon as possible, what are the options available to me? I hate having prostatectomy to remove the prostate with a long healing time and more effects side, I know that the treatment of brachytherapy are cryosurgery, or HIFU treatments, etc. I do not know what the long term has proved a better and fewer side effects, if someone needs to have personal experiences to guide me. Any input would be greatly appreciated. a big party of Thanksgiving!

Hello Mr. Tao Tao, frankly, it's a difficult question to answer for the fact that each person is different, which includes age, desires and needs, etc. In the end, should be properly evaluated by a physician. As a physician who can only advise on the various treatments available and the advantages and disadvantages of each. Ultimately this is something should have a long conversation with a urologist meeting (not as a family physician, internal medicine, etc.). There are also issues to consider with regard to treatment, as if the tumor is still in the capsule of the prostate or not, if there are metastases, etc. So, I'll give you a brief summary of the surgery, brachytherapy and hormone treatment. However, before proceeding please note the following about PSA. This protein is produced specifically by prostate cells (Its function real is to liquefy semen among others). You can do a number of conditions, including infection of the prostate, the prostate is touched, ejaculation, and finally in the prostate cancer. The absolute number of PES is not as useful as a diagnostic marker. However, a sudden increase is an indicator of good prognosis. PSA is measured by the free and bound (bound to protein in the blood). It is the combination of the two gives you a better idea what is happening. In Moreover, only a biopsy can diagnose cancer and clearly not the level of PSA, as already said the number is a better prognostic indicator of death and effectiveness of treatment. Surgery is usually used in younger patients (by young I mean 75 or 80 years in which the probability of dying something other than the prostate cancer is likely), it is still within the prostate (in other words, the tumor has not grown beyond boundaries of the prostate). In this case, removal or destruction of localized tumor is usually curative IF AND ONLY IF the whole tumor is removed. The Common side effects are of major incontinence and impotence. If you are under 50 years old when the surgery is very unlikely to recover sexual function, but before sexual function before surgery is also important. Note that the accidental cutting of nerves in results surgery loss of erection and loss of sensation or without ejaculation, drugs are widely available to assist in the erection. The advantage HIFU is recovering fast, according to some studies, a lower incidence of complications mentioned above. Radiation can be external (beam) or internally (brachytherapy) in that small radioactive "seeds" are planted in the prostate. Studies have shown that 15-30% of side effects such as burning during urination, bloody urine, frequent urination, rectal bleeding, rectal discomfort or diarrhea during or shortly after treatment. Impotence is a common side effect that often gets worse over time. healing results are the same after 10 years for both radiotherapy and surgery. A major problem with this method is not the prostate removed and examined by a pathologist. The same is true for the lymph nodes, for many urologists who is in disgrace. Incontinence is extremely rare after radiotherapy. The surgery, however, can give you a better chance of long-term cure. Hormone therapy is used to slow the growth of prostate and is used in patients in whom surgery is not just an option. It's just one last attempt to win more years of life. Some of the more serious side effects are loss of sexual desire, bone weakness, erectile dysfunction, fatigue and osteoporosis. Hope this helps, Dr. Dan
