I need help to solve a mysterious disease and the diagnosis of my father from the Dominican Republic does not seem to get it.?

I know you can not expect a diagnosis online, but we hope at least get help on where to turn. History: It is a healthy 58yo white male, no history of allergy or psychiatric illness. It is about 6 ', 185 pounds and until this mystery has been healthy.
